Hackers can modify game files to create items that are not obtainable in the actual game, in short, cheated items, (Like +15 Weapons, even if the game files had those but never implimented) And basically, whenever you log in or out of Dark Souls, the server tracks your inventory and Soul Memory.
Usually it's best not to take chances and to simply enjoy the game in it's natural state, also, the idea of hackers giving hacked items, basically: Modding in Dark Souls is a rather restricted and a rather big No-Zone if you have common sense, even the "visual" mods. On further examination, instead of running backwards, your copy of Dark Souls II seems to constantly register non-existent backwards movement of the controller.

On a side note, any great mods you folks would recomend installing? Not sure if there's anything like PVP Watchdog for this game since i think messing with dll's will get you banned. While fighting the DLC boss, Raime, if you are cornered into the left side of the darker fog gate, you will then start to redundantly run backwards without stopping. The only modding I'm unsure to be safe would be using something like cheat enguine to hack in some of those nice looking promotional reskins. Not sure if this is still a thing anymore, but how safe is it to mod this game? I'm mostly into visual mods that change model textures or just improve how the game looks (would love to make the game's lighting like the beta demo).
